About Us
The Brook Support Services based in Hertfordshire provides semi-independent supported living accommodation for vulnerable young people and care leavers aged 16 – 18yrs old (can be extended past 18 if required). Our objective is to provide a ‘stepping-stone’ between care and the move on to total independence. The service promotes safety, emotional well-being, community integration and appropriate self-determination.
We aim to provide a service that enables the young person to, as far as possible, experience what they will encounter when living fully Independently. We offer bespoke client-centered services by offering targeted and sustainable accommodation and support services for vulnerable young people.
The level of support offered to each young person varies and is regularly reviewed to suit the young person’s needs. All our Leaving Care and Supported Living accommodation are staffed 24/7.

What We Do
The Brook Support services provides a high standard of accommodation, and have a dedicated multidisciplinary staff team who are committed to supporting young people to develop the social, emotional, and practical skills they will need to enable them to make a successful transition from care to independent living, and become responsible members of their own community, and wider society.
Young people living away from home; foster care; or residential provisions for the first time often lack the practical life skills they need to be able to achieve and sustain independent living. Alongside this, some young people are very often dealing with issues of loss and transition, complicated further by mental health and substance misuse problems. At The Brook Support services each young person is provided with stable quality accommodation, and given a tailor-made support package delivered by qualified workers with specialist knowledge. The Brook Support services focus is to support the young person to address any issues identified that may be impacting on their welfare, whilst equipping them to become independent young people, making positive lifestyle choices.
The Brook Support services put no set time frame for when a young person will be ready to live independently. We will support and encourage young people to work through their Get Ready for Adult Life Pack. The key areas covered include:

The Brook Support Services will work in partnership with local authority commissioners to provide sufficient accommodation, and develop tailored packages of support to meet the needs of the individual young person.

Individual & shared accommodation with floating support for young people who are deemed ready to “move on” from 24/7 properties

Accept emergency referrals
Accept emergency referrals, alleviating the need to use unsuitable temporary accommodation.
Assess the young person’s baseline support needs
Assess the young person’s baseline support needs and create an Individual Support Plan to assist their development.
Work with social worker/personal advisor
Work with the young person’s social worker/personal advisor to develop a clear and relevant pathway plan.
Set clear boundaries about behaviour
Set clear boundaries about behaviour to ensure that young people are kept safe.
